paper works, but it cannot add up. rox is the free ios app these worksheets come from. it remembers every entry, reads your apple watch, oura, garmin or amazfit, and turns months of logs into a summary a doctor can read in a minute.
medication list template
a clean list of everything you take with dose, timing and what it is for. the one page worth carrying.
0 to 10 pain scale chart
what each number on the pain scale actually means, plus a printable pain log to keep your ratings consistent.
symptom tracker template
a daily log for what you felt, how strong it was and what helped. print it for the fridge.
mood tracker template
mood logged next to sleep and energy, so you can see what is actually moving it.
appointment prep sheet
one page to fill in beforehand so a ten minute appointment goes further.
flare log template
what came before a flare, how long it lasted and what helped. recovery time included.
blood pressure log
readings with time, position and pulse, including room for lying and standing orthostatic readings.
food diary template
what you ate, when, and what followed. the delay column is the one that finds your patterns.

bedtime, wakings, quality and how the next day felt. two weeks is what most clinics ask for.
headache diary
when it started, how long, severity, triggers and what you took. built for migraine appointments.
medication schedule template
a weekly grid of what to take and when, so you can tick doses off instead of wondering.
medical history form
diagnoses, dates, surgeries and allergies in one place, so you stop rebuilding it from memory.
01
are these worksheets really free?
yes. every worksheet here is free to use, print and share, with no email required and no account to create. we would rather you find them useful than trade your details for them.
02
do i need to print them?
no. each one can be printed or saved as a pdf from your browser, and every worksheet also has a free interactive version that does the same job on screen and works the patterns out for you.
03
can i use these with my doctor?
yes, that is largely what they are for. bring the filled-in sheet to your appointment. a page of dates, numbers and what you have already tried is far harder to dismiss than a description from memory.
04
which worksheet should i start with?
the symptom tracker if you are not sure, or the appointment prep sheet if you have a visit coming up. pick one. starting three at once is the most reliable way to abandon all of them.
05
can i share these with my support group?
please do. they are free and condition-agnostic, and sharing them costs you nothing. a link back to this page is appreciated but not required.
every worksheet here has a free interactive version that saves on your device and works out the patterns for you.
free to use, print and share. rox is a tracking tool, not medical advice, and it cannot diagnose or treat anything.
